Insurance and Billing
We are contracted with most major insurance plans. Please contact your insurance company if you are unsure of your policy and benefits. If we participate with your insurance, we will file your claim. However, for us to file your claim you must present your insurance card at each visit. Please be prepared to make your co-payment and deductible at the time of service. If we do not participate with your insurance, please be prepared to pay in full at the time of service. Payment plans are available if made in advance.
It is our policy to collect past due balances prior to providing routine services. If you have a problem paying your bill, please contact our patient account representative to set up a payment plan. We are always willing to work with anyone who shows good faith in meeting responsibilities. Finance charges may be applied to delinquent accounts.
There is a $10 charge for completion of forms and for the production of letters. This fee is due from the patient prior to the release of the form or letter. These forms include but are not limited to Disability, FMLA, Leaves of Absence, Travel, Use of Medication, Employment and Merck Medco requests.
We accept cash, checks, money orders, Mastercard and VISA. There is a $25 fee for returned checks.
